HOME AND COMMUNITY SERVICES AREA SUPERVISOR

DEFINITION

This is professional supervisory work coordinating the activities of staff responsible for providing home and community services (HCS) to elderly and disabled clients of the Department of Health and Senior Services.

This description may not include all of the duties, knowledge, skills, or abilities associated with this classification.

EXAMPLES OF WORK

Supervises social service workers, public health nurses, and/or specialized staff in diverse activities, including investigation, nursing, case management, Title XIX, and state in-home programs, hospital liaisons, relocating nursing facility residents, Residential Care Facility/Personal Care (RCF/PC), Employee Disqualification List (EDL), and Pre-Long Term Care Screening (PLTCS).

Directs the identification, investigation, and intervention of abuse/neglect/exploitation, client assessment, case management, and Pre-Long Term Care Screening to elderly and disabled clients.

Oversees and provides guidance in the development and implementation of care plans through case management for elderly or disabled clients with severe chronic health conditions which put them at risk of nursing facility placement.

Coordinates and evaluates the activities of HCS staff in an assigned geographical area of the state.

Develops resources, identifies needs or problems, and assists staff in developing and implementing new programs and policies.

Convenes interdisciplinary teams involving medical and professional staff to meet complex or specialized medical and social needs of clients.

Trains, supervises, and assures staff accountability in investigating reports of abuse/neglect/exploitation and providing protective services to elderly and disabled clients.

Assures that reasonable referrals for guardianship/conservatorship, involuntary detention, and criminal prosecution are made to the appropriate entities or officials such as probate court, law enforcement, prosecuting attorney, county public administrator, and Department of Mental Health.

Maintains fiscal, program, and quality assurance controls of in-home, personal care, and adult day health care programs and providers.

Conducts group and individual training sessions for case management functions, protective services workers, investigators, nurses, long-term care specialists, and social work practitioners.

Provides technical assistance to community agencies, providers, and the public in the areas of regulations and policies related to direct and purchased services and providers.

Exercises significant independence and initiative in the performance of responsibilities; receives general administrative direction.

Performs other related work as assigned.

KNOWLEDGE, SKILLS AND ABILITIES (KSAs)

Comprehensive knowledge of case management methods, principles, and techniques for the elderly and disabled with severe medical and protective service needs.

Comprehensive knowledge of investigative techniques and state law pertaining to abuse/neglect/exploitation of elderly and disabled individuals, and appropriate protective interventions.

Comprehensive knowledge of the legal and judicial process pertaining to guardianship/conservatorship, involuntary detention, and criminal prosecution.

Comprehensive knowledge of community resources available to elderly and disabled populations.

Intermediate knowledge of diseases, medical treatments, and complications affecting elderly and disabled populations as well as knowledge of mental illnesses and developmental disabilities.

Ability to supervise, train, coordinate, and evaluate the work of staff with diverse functions and expertise.

Ability to establish and maintain effective working relationships through their staff, with clients, health care professionals, law enforcement agencies, home health agencies, public administrators, other community agencies, and the public.

Ability to communicate effectively.

Ability to exercise judgment in evaluating situations and making decisions.

Ability to assist in planning and implementing new programs, initiatives, and/or policies relating to at-risk elderly or disabled persons.

EXPERIENCE AND EDUCATION

(The following requirements will determine merit system eligibility, experience and education ratings, and may be used to evaluate applicants for Missouri Uniform Classification and Pay System positions not requiring selection from merit registers. When practical and possible, the Division of Personnel will accept substitution of experience and education on a year-for-year basis.)

Three or more years of experience as a Social Service Worker, Long-Term Care Specialist, or Public Health Nurse with the Missouri Department of Health and Senior Services.

OR

A Bachelor's degree from an accredited college or university with a minimum of 15 earned credit hours in Social Work, Gerontology, Criminal Justice, Nursing, social science, Health Care Administration, Special Education, Psychology, Sociology, Counseling, or Human Services; and,

Three or more years of experience in a public or private agency in the delivery of social work or protective services for children, the elderly, or persons with developmental disabilities or mental illness.

(24 earned graduate credit hours from an accredited college or university in the specified areas may substitute for one year of the required experience.)
