OneForm Instructions to Download CARO Forms

1. Print a copy of this instruction page.
2. Click on the setup file (32 Bit Init Setup) below.

3. At the File Download Window, select option to "Save this program to disk" and click "OK".
4. At the Save As Window, choose to download the setup file to the c:\temp folder.
5. When download is complete, minimize this web page.
6. Open your Windows Explorer, find the c:\temp folder and double-click on it.
7. Find the setup file you just downloaded (32bitdeforminit7.exe) and double-click it to run the file.
8. A small window will display, click "Yes" to run the setup.
9. At the Eform Initialization Welcome Window, click "Finish".
10. As the program loads necessary files, click "Next" or "Finish" when prompted.
11. After setup is complete, please click the "Registration Form" button, complete the registration form and submit it. We will use this information to contact you about future updates to the electronic forms.
12. You're done here! Move on to downloading the specific CARO form.
