Forms Services

Step 1: Download the Setup Program

Click to download the setup program

Click this button to download the setup program. Choose the "Run" or "Open" option and follow the windows. The setup program is required to use any of the electronic forms below.

Step 2: Find and Download a Form

Find a Form by:

Find a form by title

Click this button to find a form by title, partial-title or keyword. The form number cannot be used as a title or keyword. The search window may take a few seconds to load. Pop-up stoppers must be disabled.

Find a form by agency

Click this button to find a specific agency or group. Click on the agency or group link to view its form titles.

Download the form:

  1. Click the Form Title.
  2. At the download window, click the Run or Open button.
  3. If you get an Internet Explorer Security Window (Unknown Publisher), click the Run button.
  4. Click the Next button on the first three windows that display.
  5. Click the Finish button on the last two windows that display.
  6. Download is complete, exit the Intertnet.
  7. See Step 4 below for information on how to open and use the forms.

This information is used to contact you in case of future updates.  Thank you for registering!

Click this button to register as an electronic form user. This information is used to notify registered users of any major changes to the electronic forms.

Step 4: Using the EForms / Eform Features

  1. Click the Start button.
  2. Click on Programs.
  3. Find the OneForm shortcut for the agency or group.
  4. Mouse over the shortcut and click on the form title that is displayed. The form should open.
  5. Type data into the green field and click the Tab button to move to the next field/next page. Check boxes can be filled by clicking the Space Bar or selecting with the mouse.
  6. Double-click Date fields to display a calendar.
  7. Click the Save button at top of form to name and save file.
  8. To reopen a previously saved file at a later date, open a blank form (Items 1 through 4 above) and select File and Open from the menu.
