Electronic Forms

Forms Services has created electronic versions of many State of Missouri's paper forms by using software called OneForm by Amgraf.

These electronic forms, or "eforms", are identical to their paper counterparts and can be filled, saved and printed. Eforms allow us to provide electronic forms without the expense of the user having to purchase a filler package. What is required to use the eforms?

1. Windows operating system (Windows 95, 98, NT, 2000 or XP)
2. OneForm setup file (This file only needs to be downloaded and ran once)
3. Individual form files

Once the setup is completed, customers can download the eforms individually or by agency groups. The forms are listed by agency first and then alphabetically by title on the Eforms Download Page.
