Frequently Asked Questions
Any state employee, contractor, lobbyist, or member
of the press located
in a state owned building maintained by the Office of Administration,
Division of Facilities Management, Design and Construction
(FMDC) is required to have a State ID Badge. Departments with
staff or contractors located in leased facilities or those
not maintained by FMDC may also request badges.
If a badge holder leaves a department’s employ and/or transfers
to another state department, the current badge should be returned
to the appropriate processing location for inactivation. These
badges will then be destroyed by FMDC.
All contractor badges that are no longer needed or have expired
must subsequently be returned to FMDC for inactivation and shredding.
AA – Appointing Authorities: Usually
the department director or a person who has authorization to
designate individuals they wish to represent the department.To

SA – Signature Authorities: Department
individuals authorized by the department’s authorized authority
to sign applications for ID badges with or without secured
area access for employees and contractors who may be working
with a department. These
individuals may also send requests for badge reprints or access
revisions via email to the appropriate processing
location. Requests
for new security accesses for employees that have an existing
ID Badge will need to be submitted via the application with
the secured area access information included.
If a department has employees that are not located
in close proximity to a processing location, they may submit
employee photographs to FMDC on a CD or by email attachment. Hard copies of the completed
applications signed by the department’s signature authority and
photocopies of current photo identifications of each employee must
be submitted before processing can be completed. Due to poor
quality, FMDC asks that forms of photo identification not be
submitted electronically.
If the photographs are not compatible
to the ID Badge system, FMDC will notify the department that they
must have their employees travel to the nearest processing location.

identification not be submitted electronically.
Department Signature Authorities may send requests for badge reprints
or access revisions via email to the appropriate processing
location.
Requests for new security accesses for employees

etc.) to the appropriate location. The information will be
verified and an ID badge will be processed.

FMDC asks that requests for State ID Badges with building and/or
secured area access be submitted prior to sending the employee to
the processing location.Upon receipt of the application signed by
the department’s signature
authority, a report run through Capitol Police for outstanding wants
or warrants will be requested. Results of any outstanding warrant
may result in the individual being contacted by Capitol Police
or local law enforcement agency.

department will be notified that the employee should report to
the appropriate location for badge processing. Upon arrival,
the state employee must provide a form of current photo identification
(driver's license, nondriver license photo ID, military ID, minor
ID, etc.) Their State ID Badge with building or secured area access
will then be processed. Please note that the St. Louis, Kansas
City and St. Joseph processing stations issue separate cards for
ID Badges and Access Cards.
Once the application is approved, FMDC will notify the department
that the employee should report to the appropriate location for processing.

for State ID Badges with building and/or secured area access be submitted
prior to sending the employee to the processing location.

Badge along with a current criminal background check through
the Highway Patrol be submitted prior to sending the contractor
to the processing location. A form to request a Missouri State
Highway Patrol Criminal Background Check for contractors working
for State Departments can be found on the web at: www.mshp.dps.missouri.gov/MSHPWeb/PatrolDivisions/CRID/CrimRecChk.html If
a contractor does not have a social security number or a valid
driver’s license or has been living in the state for less than
one year, a copy of another form of photo identification should
be attached to the application that includes the former resident
address. If
the contractor will be permanently located in a building indicate
where in the area designated for “Building, Floor, Room Number” on
the application.
Upon receipt of the application signed
by the department’s signature
authority, a report run through Capitol Police for outstanding wants
or warrants will be requested. Results of any outstanding
warrant may result in the individual being contacted by Capitol
Police or local law enforcement agency.
Unless specified by the host department for a shorter
period of time, all Contractor’s ID Badges will expire one
year from date of issuance.
If a contractor continues to need an ID Badge, the host department
will need to submit a completed application along with a current background
check provided through the Highway Patrol as outlined in the procedures.
It is the host department’s responsibility to ensure that contractors
have current Contractor ID Badges. In an effort to assist departments,
FMDC will provide monthly reports showing contractor badges
due to expire within the next 30 days. On the date of expiration,
Contractor Badges will be inactivated until FMDC receives a completed
ID Badge Application signed by the department’s signature authority,
a current background check through the Highway Patrol and the
new badge is properly processed.
